Required Forms

These forms are due 30 days prior to the start of your program. Turning in these forms on time allows us to best prepare for your program.  If you have any questions about these forms or your program in general, please contact the program director at 209-379-9511 ext. 11.

 

Pre-Trip Questionnaire 

Our staff needs your completed pre-trip questionnaire in order to develop a teaching plan for your week.

 

Teaching/Trail Group Lists with Medical/Dietary Alerts 

Information on your teaching/trail group lists and medical/dietary alerts corresponds to your participants' completed registration forms. List all medical alerts, medical conditions, and dietary restrictions that you find listed on your participants registration form (health problems, allergies, recent surgeries, injuries or illnesses, medications, etc.) Use this time to check registration forms for signatures by parent or legal guardian. All forms must be signed.

Remember our policy is that only two adults go out on trail with each group. It is important for you to bring as many adults as you feel will provide adequate supervision for your students, with advanced planning, but extra chaperones can not go out on trail.  Extra chaperones can, however, be a great help with supervision during free time.

Please contact the program director if you are unsure of the number of teaching groups that you will have. We aim for an average of 12 students per hiking group. The number of instructors assigned to your school may decrease if your youth numbers decreased. 

 

Curry Village Rooming List

Groups staying in Curry Village must complete a rooming list. We cannot guarantee accommodations for your group if we do not receive this form on time.

During tent cabin season, the location of each of your group's cabins may change due to the configuration of the tent cabin area. If this happens, we will e-mail you.
